Implementation Log
Use this log to track one accessibility action over time. Record what you tried, what changed, what got stuck, and what needs to happen next.
How to use the log
Start with one action from your Accessibility Action Plan. Keep the record simple and practical. The log is designed to help you notice progress, barriers, and next steps.
1. What action did you choose?
Name the accessibility action you are tracking.
2. What has been tried?
Record what has happened so far. This could include a test, conversation, change, review, update, or first step.
3. What changed?
Describe any improvement, feedback, outcome, or learning from the action.
4. What barriers came up?
Record anything that made implementation harder, including time, tools, confidence, unclear responsibility, technical barriers, or access needs.
5. What needs to happen next?
Choose one next step. Keep it realistic and specific.
6. When will you review this again?
Set a date or review point to check progress.
Next step
After recording progress, choose whether to request advisory support or continue to the next-steps guidance.
