Refer a student for support, assessment or a classroom pilot
Use this form to refer a student for EduLinked supports — classroom pilots (EMLS), assistive technology assessments (AT-EMLS), training for staff, or tailored inclusion plans. We will treat personal information respectfully and provide Easy Read or Auslan materials on request.
If you prefer an Easy Read or Auslan video guide, tick the box in the form or email founder@edulinked.com.au. If the student is at immediate risk, call local emergency services rather than using this form.

- Your name & role: who is making the referral.
- Student: name or initials, age/year, school and class.
- Support needs: write the main support the student needs.
- Guardian contact: include if you can — we will ask for consent if needed.
- Request: tick what support you want (pilot, AT assessment, training, etc.).
- Consent: tick the consent box to confirm you can refer the student.

What happens next
Initial response
We aim to contact the nominated school or guardian within five business days to confirm details and next steps.
Consent & approvals
If the referral involves identifiable student data or research, we will request parental/carer consent and provide templates for that process.
Pilot planning
If a pilot or assessment is appropriate we will prepare a partner brief, MOU and operational plan for approval.

- For identifiable student data or research, yes. For initial scoping we may proceed with school-level permissions — EduLinked will advise during follow-up.
- How long until you contact us?
- We aim to respond within five business days. If urgent, call local emergency services for safety matters, or phone our team (see below).
- Can we submit anonymously?
- Anonymous referrals are possible for sensitive cases — contact us by email to discuss safe referral options.
