Assistive Technology Referral

Use this form to ask for a short assistive technology (AT) trial or advice. This helps with AAC, switches, symbol software, eye-gaze or device setup.

In short

We check needs, confirm consent, and arrange a short trial or scoping session. We give clear trial instructions, training and a short outcomes summary.

Students or learners who need communication or access supports
People needing device or software trials
School staff, clinicians, families or community workers

What we do

Check goals and functional needs
Arrange short trials (e.g., 2–6 weeks) where suitable
Configure devices and provide short training
Give a trial report and procurement notes if needed
