Refer your client (NDIS participants and support coordination)
Support coordinators, plan managers, allied health and providers can refer NDIS participants for EduLinked supports — including navigator tools, plan review support, access & communication toolkits and assistive-technology scoping.
If your referral is urgent for a plan review or meeting, email founder@edulinked.com.au with the subject “Urgent referral”.
Overview
Use this form to refer an NDIS participant for support navigation, plan preparation, provider-readiness checks, or assistive-technology scoping. Provide participant details, NDIS plan/funding info where available, and a short summary of supports requested.
Process
We confirm consent and supports needed, undertake a short co-design or scoping activity, and deliver a toolkit or short report to support plan reviews, provider engagement or access improvements.

- Ensure an authorised decision-maker provides consent; record their details and relationship in your referral notes.
- Can you support plan reviews?
- Yes — we can prepare plain-language summaries, evidence packs and guidance to support plan reviews and funding discussions.
